Award Criteria
Nominations will be judged on the strength of their submission by the following criteria:
  1. Participation
    • The nominee must be a registered athlete in a Special Olympics sport club.
    • The nominee must be involved in Special Olympics sport program(s) throughout the year
  2. Athletic and Personal Development
    • The nominee must be successful in his/her sport, doing their personal best to achieve their athletic goal.
    • The nominee must demonstrate improvement in athletic skill.
    • The nominee must demonstrate improvement in personal growth and development.
  3. SPORTSMANSHIP
    • The nominee must exemplify the true spirit of Special Olympics.
    • The nominee must display sportsmanship and be considerate of fellow athletes, coaches and officials.
    • The nominee must consistently act as a role model for other Special Olympics athletes.
Please complete this form by Friday, May 18, 2018. Please note that any submissions received after this date will not be considered. Please use a separate form for each nominee. There is no limit to the number of athletes you may nominate. Special Olympics Ontario will present the awards at its Annual General Meeting and will nominate all winners for national recognition to Special Olympics Canada.

  2. Athletic and Personal Development
    • The nominee must have demonstrated success in his/her sport(s), doing their personal best to achieve their athletic goals.
    • The nominee must have demonstrated a dedication to improve their athletic skill over the course of their career.
    • The nominee must have demonstrated an improvement in personal growth and development over the course of their career.
  3. Sportsmanship
    • The nominee must exemplify/have exemplified the true spirit of Special Olympics.
    • The nominee must display/have displayed sportsmanship and consideration of fellow athletes, coaches and officials.
    • The nominee must/have consistently act/acted as a role model for other Special Olympics athletes.
  4. Additional Considerations
    • Please list any other significant achievements the nominee has achieved outside of their athletic accomplishments (i.e. International Global Ambassador, Board Member, etc.)
