You may additionally download the registration package below, and go through the process outlined in the steps below to submit both your registration forms as well as your police check.
REGISTRATION PACKAGE:

- Click on Volunteer Registration Form
- The form will be displayed in a PDF format
- Print this form and return it with the Brockville Police Check form (see below) to the Volunteer Coordinator
POLICE CHECK FORM
- To obtain the Brockvill Police Check form please click on the following link: http://www.brockvillepolice.com/services/documents/BPSConsentDec09.pdf
- The form will be displayed in a PDF format
- Print this form and take the completed form to the local Brockville Police station to process your Police Records Check
- Once you have received the Police Records check back please provide it with the Volunteer Registration Form to the Volunteer Coordinator.
There is a cost for the Police Records Check. However, once it has been approved please provide the Volunteer Coordinator with a copy of the receipt of payment to the Brockville Police for reimbursement.
